Welcome

Parents or guardians often consider a psychoeducational assessment for their child when the child experiences challenges at school. School is not “working” as it should. A child may be struggling academically, socially, and/or emotionally.

Parents or guardians of children or adolescents, or adults with a disability may need a psychoeducational assessment to assist them in accessing appropriate services.

My goal in doing a psychoeducational assessment is to get the best data available from many sources; the child, youth, or adult; the parents; the teachers or support workers; and medical personnel (if applicable) to discover the individual’s strengths and weaknesses. From this information, recommendations can be made to improve wellbeing.

If you are seeking a psychoeducational assessment you will need to complete the following forms before services can be provided:

You may download these forms and bring them with you to the appointment or they will be provided at the time of the appointment. The information in the forms will be discussed at your initial appointment and may be revisited at any time during treatment.
