Psychoeducational Assessment

Psychoeducational assessments can be used for clinical and educational decision-making, for planning treatment, and for planning remediation. Educational personnel may use the information from psychoeducational assessments to help determine a student’s eligibility for special services while in school or for transitioning into adulthood. Mental health personnel may use this information to establish effective treatment plans and to advocate for needed services for clients with special needs.

A psychoeducational assessment for a school-aged child or youth will include the following items:

  • A client history questionnaire, which will include relevant family, developmental, social, emotional, educational, and health history of the child or youth.
  • Wechsler Intelligence Scale for Children – 5th edition (WISC-V). The WISC-V is a test of ability for children.
  • Wechsler Individual Achievement Test – 3rd edition (WIAT-III). The WIAT-III is a test of achievement.
  • Behaviour Rating Inventory of Executive Function (BRIEF)—parent, teacher, & self-rating scales. The BRIEF is a behaviour checklist for parent, teachers, and the child or youth to complete.
  • Behaviour Assessment System for Children, Third edition (BASC-3)—parent, teacher & self-rating scales. The BASC-3 is a behaviour checklist for parent, teachers, and the child or youth to complete.
  • Other assessment instruments may be used as required.

After all data has been collected and testing has been completed, the psychologist will synthesize the information and create a written assessment report. Key findings will be discussed as well as recommendations for follow-up. A follow-up debrief of the assessment report will be completed at the clinic.

A psychoeducational assessment for an adult with a disability will include the following items:

  • A client history questionnaire, which will include relevant family, developmental, social, emotional, educational, and health history of the youth or adult.
  • Wechsler Adult Intelligence Scale – 4th edition (WAIS-IV). The WAIS-IV is a test of ability for adults.
  • Adaptive Behaviour Assessment System, 3rd edition (ABAS-3). The ABAS-3 is an assessment of the daily adaptive behaviours of the individual being assessed.
  • Other assessment instruments may be used as required.

Fees

Psychoeducational assessments that include the following items: client history questionnaire, WISC-V, WIAT-III, BRIEF, AND BASC-3 are billed at a rate of $1600.00. There may be an additional charge if other test instruments are required. This will be discussed in advance of testing.

  • Payment must be made before services are rendered.
  • Payments can be made by cash or Paypal.
  • If you plan on being reimbursed by insurance you are responsible to submit the receipt to your insurance company.

Psychoeducational assessments for an adult with a disability that include the following items: client history questionnaire, WAIS-IV, and ABAS-3 are billed at a rate of $800.00. There may be an additional charge if other test instruments are required. This will be discussed in advance of testing.

Cancellation Policy

Missed appointments or cancellations with less than 24 hours notice will be billed at a rate of $200.00.
